New Genetic Discovery May Eventually End Premature Birth

Researchers have found genetic mutations that affect whether a woman is likely to have her baby early or carry it to full term. 

Even late preterm babies, those born between 34 and 36 weeks of gestation, are more likely to die or experience problems, even if they are the size and weight of some full-term infants born after 37 to 41 weeks in the womb. 

Preterm birth is the leading cause of death among children younger than 5 worldwide. These babies have higher death rates even into adolescence and beyond.

Several studies show health problems related to preterm birth persist through adult life, problems such as chronic lung disease, developmental handicaps, vision and hearing losses. The World Health Organization reports that every year, an estimated 15 million babies are born early, and this number is rising. Until now, little was known about the causes, but these findings could help solve the mystery.

Beginning of a journey

Dr. Louis Muglia coordinated the study of the DNA of more than 50,000 pregnant women. The study identified six gene regions, which influence the length of pregnancy and the timing of birth. While the study doesn’t provide information about how to prevent prematurity, Muglia says it could eventually do that. 

“It’s just the beginning of the journey, but at least we know now, what the foundation is,” he says.

Muglia is co-director of the Perinatal Institute, which focuses on preterm babies, at Cincinnati Children’s Hospital Medical Center. He’s also the principal investigator of one of the March of Dimes’ five prematurity research centers. The March of Dimes helped pay for the study along with the National Institutes of Health, the Bill and Melinda Gates Foundation and other medical research institutes.

Muglia said scientists have known for a long time that preterm birth is a combination of genetic and environmental factors. This study showed the genes involved were from the mother. 

“For the first time, we have an idea of what tissue in the mom is the one that is likely driving the one for preterm birth,” Muglia says.

Selenium

One of the genes identified is involved in how the body uses selenium, a common mineral provided in food or supplements, but not currently included in vitamins women commonly take while pregnant. Selenium supplements are low-cost, and if the results are confirmed, this supplement could save millions of lives. Supplements including folic acid have been shown to greatly reduce birth defects, so much so that food in many countries is fortified with this particular B vitamin.

Another gene indicated that cells that line the uterus play a larger-than-expected role in the length of pregnancy.

The researchers were from the U.S. and from Norway, Denmark, Finland and Sweden. They only tested women of European descent, so more work needs to be done involving women of other races and ethnic origins. 

But their study does open up areas for researching potential diagnostic tests, new medications, improved dietary supplements or other changes that could help more women have full-term pregnancies, all areas which will require several more years of study.

The study was published in the New England Journal of Medicine.

UN Environment Head: Asia Must Lead Charge for Pollution-free Planet

Asia-Pacific — home to more than half the world’s population and some of its fastest-growing economies — is a key battleground in the fight against pollution, one of the biggest threats to the planet and its people, the U.N. environment chief said.

An estimated 12 million people die prematurely each year because of unhealthy environments, 7 million of them due to air pollution alone, making pollution “the biggest killer of humanity,” Erik Solheim told the first Asia-Pacific Ministerial Summit on the Environment in Bangkok this week.

Humans have caused pollution and humans can fix it, said Solheim, executive director of UN Environment, in an interview with Reuters at the four-day summit.

“The struggle for a pollution-free planet will be won or lost in Asia — nowhere else,” said the former Norwegian minister for environment and international development.

The sheer size of Asia-Pacific, as well as its continued economic growth, put it at the heart of the challenge, he added.

The region’s development has been accompanied by worsening pollution of its air, water and soil. Its emissions of planet-warming carbon dioxide doubled between 1990 and 2012, and the use of resources such as minerals, metals and biomass has tripled, according to the United Nations.

World Health Organization figures also show Asia has 25 of the world’s 30 most-polluted cities in terms of fine particles in the air that pose the greatest risks to human health. The pollution comes largely from the combustion of fossil fuels, mostly for transport and electricity generation.

Solheim said Asia is also a major contributor of plastic polluting the world’s oceans — and solutions can be found in the region. He pointed to a huge beach cleanup campaign in Mumbai that inspired Indian Prime Minister Narendra Modi to overhaul the country’s waste management system.

“There’s enormous environmental opportunity,” Solheim said. “Asia has by and large strong governments, and they have the ability to fix problems.”

Coal no longer king?

Solheim said fighting pollution by moving toward renewable energy sources such as wind and solar would also benefit efforts to curb climate change, which scientists say is stoking more deadly heatwaves, floods and sea-level rise around the world.

But environmentalists worry that Asia’s demand for coal, the most polluting of the major fossil fuels, is likely to grow for years to come.

Figures from a forum organized by the King Abdullah Petroleum Studies and Research Center in Singapore earlier this year show that some 273 gigawatts of coal power are still being built, although much more has been put on hold.

In July, analysts told Reuters that Japan, China and South Korea are bank-rolling coal-fired power plants in Indonesia despite their pledges to reduce planet-warming emissions under the Paris climate deal.

The landmark 2015 Paris Agreement seeks to limit the rise in average world temperatures to well below 2 degrees Celsius above pre-industrial times. Experts say curbing or ending the use of coal is required if this goal is to be reached.

Globally, many countries — including China — are shutting down or suspending plans for coal-fired power plants as costs for wind and solar power plummet.

Solheim is optimistic, noting that the International Energy Agency significantly raised its five-year growth forecast for renewables led by China, India, the United States and Mexico.

“There are very, very few people in the world who believe that the future is coal,” he said. “I think we will see the shift [to renewables] happening much faster than people tend to believe.”

On U.S. President Donald Trump’s decision to pull his nation out of the Paris Agreement, Solheim sees a silver lining.

“The surprising judgment of history may be that Donald Trump did a lot of service to this fight against climate change by withdrawing, because he galvanized the reaction of everyone else,” said Solheim.

“All the big, iconic companies of modern capitalism — Apple, Google, Microsoft, Amazon — they immediately said, ‘We will move into the green economy.'”

Studying Black Holes in a Bathtub

Mysterious black holes, thought to reside in the center of every galaxy, are difficult to study because even the closest one, in the center of our own Milky Way, is still some 27,000 light years away. But researchers at the University of Nottingham’s Quantum Gravity Laboratory have found that some of the physical phenomena linked to black holes can be studied in an ordinary bathtub. ‘Reprogrammed’ Stem Cells Fight Parkinson’s Disease in Monkeys

Scientists have successfully used “reprogrammed” stem cells to restore functioning brain cells in monkeys, raising hopes the technique could be used in the future to help patients with Parkinson’s disease.

Since Parkinson’s is caused by a lack of dopamine made by brain cells, researchers have long hoped to use stem cells to restore normal production of the neurotransmitter chemical.

Now, for the first time, Japanese researchers have shown that human induced pluripotent stem cells (iPS) can be administered safely and effectively to treat primates with symptoms of the debilitating disease.

So-called iPS cells are made by removing mature cells from an individual — often from the skin — and reprogramming them to behave like embryonic stem cells. They can then be coaxed into dopamine-producing brain cells.

The scientists from Kyoto University, a world-leader in iPS technology, said their experiment indicated that this approach could potentially be used for the clinical treatment of human patients with Parkinson’s.

In addition to boosting dopamine production, the tests showed improved movement in affected monkeys and no tumors in their brains for at least two years.

The human iPS cells used in the experiment worked whether they came from healthy individuals or Parkinson’s disease patients, the Japanese team reported in the journal Nature on Wednesday.

“This is extremely promising research demonstrating that a safe and highly effective cell therapy for Parkinson’s can be produced in the lab,” said Tilo Kunath of the MRC Center for Regenerative Medicine, University of Edinburgh, who was not involved in the research.

The next step will be to test the treatment in a first-in-human clinical trial, which Jun Takahashi of Kyoto University told Reuters he hoped to start by the end of 2018.

Any widespread use of the new therapy is still many years away, but the research has significantly reduced previous uncertainties about iPS-derived cell grafts.

The fact that this research uses iPS cells rather human embryonic stem cells means the treatment would be acceptable in countries such as Ireland and much of Latin America, where embryonic cells are banned.

Excitement about the promise of stem cells has led to hundreds of medical centers springing up around the world claiming to be able to repair damaged tissue in conditions such as multiple sclerosis and Parkinson’s.

While some treatments for cancer and skin grafts have been approved by regulators, many other potential therapies are only in early-stage development, prompting a warning last month by health experts about the dangers of “stem-cell tourism.”

Dream Chaser Spacecraft in Captive-carry Test Over Desert

A test version of a spacecraft resembling a mini space shuttle was carried aloft over the Mojave Desert by a helicopter Wednesday in a precursor to a free flight in which it will be released to autonomously land on a runway as it would in a return from orbit.

 

Sierra Nevada Corp.’s Dream Chaser craft was lifted off the ground at 7:21 a.m., at NASA’s Armstrong Flight Research Center on Edwards Air Force Base, California, and was carried to the same altitude and flight conditions it will experience before release in a free flight.

 

A control team sent commands to the wingless vehicle and collected data before the helicopter brought it down at 9:02 a.m., the company said.

 

“Everything we have seen points to a successful test with useful data for the next round of testing,” director of flight operations Lee “Bru” Archambault said in a statement.

 

A second captive-carry test is scheduled this year and if it is successful, a free flight test will follow.

 

The Dream Chaser is being developed to carry cargo to and from the International Space Station without a crew aboard. The version flown Wednesday is for tests in the atmosphere. The version that will be launched into space is still in development.

 

With the addition of life-support equipment, a Dream Chaser could transport a crew of seven.

 

Last month, Sierra Nevada selected United Launch Alliance’s Atlas 5 rocket to launch the first two Dream Chaser cargo missions, which are scheduled to blast off from Cape Canaveral, Florida, in 2020 and 2021. Those missions will land at Kennedy Space Center.

 

The Dream Chaser is a type of craft known as a “lifting body” in which aerodynamic lift is generated by its shape rather than wings like those of a conventional aircraft. Tail fins angling upward at the rear of the craft provide control.

 

NASA proved the lifting body concept by flying a series of wingless aircraft at Edwards in the 1960s and ’70s.

 

The Dream Chaser is 30 feet (9 meters) long, about one quarter the length of a space shuttle.

 

Sierra Nevada is headquartered in Sparks, Nevada, and the Dream Chaser is being developed by the company’s Louisville, Colorado-based Space Systems business.

Famed T. rex ‘Sue’ Will Get New Look at Chicago’s Field Museum

The world’s biggest T. rex is getting ready for a cutting-edge makeover.

The Field Museum in Chicago said Wednesday that it would take down and remount the 40½-foot-long (12.3-meter) Tyrannosaurus nicknamed Sue, perhaps the world’s most famous dinosaur fossil, in a way that embodies the latest understanding of this ferocious Cretaceous Period predator.

The big T. rex will move to a new exhibition space in the museum, while a cast of the skeleton of the largest-known dinosaur, Patagotitan mayorum, will take the spot Sue now occupies in the museum’s Stanley Field Hall.

Patagotitan, a long-necked, four-legged plant-eater that was 122 feet (37.2 meters) long and weighed 70 tons, lived in Argentina 100 million years ago, more than 30 million years before T. rex stalked western North America. The biggest land animal on record, it was a member of a dinosaur group called titanosaurs.

The museum next spring will unveil the fiberglass Patagotitan skeleton, which is being cast from fossils of seven Patagotitan individuals, and for two years will display some of the genuine fossils, including an 8-foot (2.4-meter) thighbone.

Named for the woman who discovered the fossils in South Dakota in 1990, Sue is the largest, most complete and best-preserved Tyrannosaurus rex ever unearthed. The museum bought the fossils at auction for $8.4 million.

Sue will be taken down in February and put up again with noteworthy changes in anatomy and stance in its new exhibition hall in spring 2019, museum scientists said.

“We are making several adjustments to the skeleton to reflect new and improved knowledge,” said paleontologist Pete Makovicky, the museum’s associate curator of dinosaurs.

The most striking change, Makovicky said, will be the addition of gastralia, bones resembling an additional set of ribs spanning the belly that may have provided structural support to help the dinosaur breathe. Adding these bones will illustrate just how massive Sue was and that it boasted a bulging belly, he added.

The scientists concluded that the bone mounted as Sue’s wishbone was misidentified in 2000, and they will replace it with the dinosaur’s actual wishbone, or furcula, the fused collarbones typical of meat-eating dinosaurs and their evolutionary descendants, the birds.

They also will adjust the ribs to produce a slimmer, less barrel-shaped chest and arrange the right leg so Sue is not crouching as much.

“Often when you do something as expensive as mounting a vertebrate fossil skeleton for display, you only get one shot at it. I’m happy we’re going to fix and update this incredible fossil,” said paleontologist Bill Simpson, who heads the museum’s geological collections.

Lifespan and bite force

Makovicky noted the accumulation of knowledge about T. rex and its cousins since 2000.

“We now know more about tyrannosaur lifespans — around 30 years; how they grew — very fast as teenagers; and using computer models of Sue, we revised their body mass upward to 9 or more tons, from 5 to 7 tons,” Makovicky said.

Ongoing research is examining the molecular composition of cartilage preserved in T. rex bones, and recent studies have shown it possessed the most powerful bite of any land animal ever, Makovicky added.

When the Patagotitan skeleton is mounted, visitors will be able to walk underneath it and touch it. Its head will reach the museum’s second-floor balcony nearly 30 feet (9 meters) up.

The museum said a $16.5 million gift from the Kenneth C. Griffin Charitable Fund, established by the founder and chief executive of hedge fund firm Citadel LLC, enabled it to carry out Sue’s makeover and add the Patagotitan. The changes coincide with the museum’s 125th anniversary in 2018.

Climate to Push Forest-eating Beetles to Northern US, Canada, Scientists Predict

Forests in the northeastern United States and southern Canada could be ravaged by tree-killing beetles in coming decades as a warming climate expands the pest’s habitat, a study has found.

Over the next 60 years, southern pine beetles could infest forests in new areas of the United States and Canada, disrupting industries and ecosystems alike, it said.

Warmer winter nights allow spread

The red-brown insects, the size of a grain of rice, known to feast on pine-tree bark, has typically only thrived in the hotter climate of Central America and the southeastern United States.

But in recent years warmer than usual winter nights have allowed it to survive the cold months and spread as far north as the U.S. state of New York. The coldest winter night has warmed by 6 to 7 degrees Fahrenheit (3 to 4 degrees Celsius) over the past 50 years in various parts of the United States, the study’s authors said.

Using computer-based climate models, they predicted the beetles should gradually march north along the Atlantic coast, infesting forests including in the U.S. states of Maine and Ohio all the way to Canada’s Nova Scotia.

Pest moves fast

By 2080, the pest should proliferate to red — and jack-pine forests in a 270,000 square miles (700,000 square km) area of the United States and Canada — roughly the size of Afghanistan, the researchers wrote in Nature Climate Change.

That would not only upend ecosystems, but also disrupt several key industries “in already struggling rural areas,” said lead author Corey Lesk, a researcher at Columbia University in New York.

“Residents of these regions could see a direct hit to their pocketbooks,” he told the Thomson Reuters Foundation on Tuesday in a phone interview.

Infestations costly to timber industry

Where the southern pine beetle has struck in the past, timber industries have been hard hit.

Infestations of pine beetles have cost an estimated $100 million a year in timber losses from 1990 to 2004 in the southeastern United States, according to the U.S. Forest Service.

Thousands of adult beetles can kill a tree in two to four months as the insects carve S-shaped tunnels under the bark, depriving their host of needed nutrients.

The tourism industry would also likely suffer, said Lesk, with the potential destruction of iconic forests including the Pine Barrens of New Jersey and Long Island.

Europe faces same problem

In Europe, previous research has shown that bark beetles have similarly been chewing through pine and spruce trees in forests from the Swiss Alps to Belarus alongside temperature increases.

Land managers have found the best way to fight off bark beetles has been thinning high-density forests and cutting out infested trees, though with limited success, the researchers said.

“The key question is whether those strategies would be able to keep up with rapid advance of the pest into regions with little or no experience managing it,” Lesk said.

 

US Spacecraft Readies for Fiery Plunge into Saturn After 13-year Mission

The U.S. space agency’s Cassini spacecraft will end its 13-year mission to Saturn in mid-September by transmitting data until the final moment before it plunges into the ringed planet’s atmosphere, officials said Tuesday.

Cassini, the first spacecraft to orbit Saturn, will make the last of 22 farewell dives between the planet’s rings and surface on Sept. 15. The spacecraft will then burn up as it heads straight into the gas giant’s crushing atmosphere.

Cassini’s final dive will end a mission that provided groundbreaking discoveries that included seasonal changes on Saturn, the moon Titan’s resemblance to a primordial Earth, and a global ocean on the moon Enceladus with ice plumes spouting from its surface.

“The mission has been insanely, wildly, beautifully successful, and it’s coming to an end in about two weeks,” Curt Niebur, Cassini program scientist, said on a telephone conference call with reporters from NASA’s Jet Propulsion Laboratory in California.

Cassini’s final photo as it heads into Saturn’s atmosphere will likely be of propellers, or gaps in the rings caused by moonlets, said project scientist Linda Spilker.

The spacecraft will provide near real-time data on the atmosphere until it loses contact with Earth at 4:54 a.m. PDT (1154 GMT) on Sept. 15, the National Aeronautics and Space Administration said.

Spilker said Cassini’s latest data on the rings had shown they had a lighter mass than forecast. That suggests they are younger than expected, at about 120 million years, and thus were created after the birth of the solar system, she said.

During its final orbits between the atmosphere and the rings, Cassini also studied Saturn’s atmosphere and took measurements to determine the size of the planet’s rocky core.

Cassini has been probing Saturn, the sixth planet from the sun, and its entourage of 62 known moons since July 2004. It has provided enough data for almost 4,000 scientific papers.

Since the craft is running low on fuel, NASA is crashing it into Saturn to avoid any chance Cassini could someday collide with Titan, Enceladus or any other moon that has the potential to support indigenous microbial life.

By destroying the spacecraft, NASA will ensure that any hitchhiking Earth microbes still alive on Cassini will not contaminate the moons for future study.

Syrian Schools Grow Edible Playgrounds to Boost Diets of Hungry Children

School playgrounds across Syria are being transformed into vegetable gardens where children whose diets have been devastated by six years of war can learn to grow — and then eat — aubergines, lettuces, peppers, cabbages and cucumbers.

Traditional Syrian cuisine is typical of the region, and rich in vegetables. Its mainstays include hummus, minced lamb cooked with pine nuts and spices, varied salads, stews made with green beans, okra or courgettes and tomatoes, stuffed cabbage leaves and artichoke hearts.

But the six-year war has changed that for much of the population, and many now live mainly on bread or food aid.

According to U.N. figures, unemployment now stands at more than 50 percent, and nearly 70 percent of the population is living in extreme poverty, in what was once a relatively wealthy country.

“The ongoing crisis in Syria is having a devastating effect on the health and nutrition of an entire generation of children,” Adam Yao, the U.N. Food and Agriculture Organization’s (FAO) acting representative in Syria, said on Tuesday, ahead of the start of the school year.

FAO is helping some 17 primary schools in both government and opposition-controlled areas to plant up to 500 meter-square fruit and vegetable plots in war-torn areas including Aleppo, Hama, Homs, Idlib and the outskirts of Damascus.

Young children are often the most vulnerable to malnutrition in a crisis, which can have serious and long-lasting effects on their growth and future development.

“Good nutrition is a child’s first defense against common diseases and important for children to be able to lead an active and healthy life,” Yao added.

The primary schools, which began planting in May, have produced 12 tons of fruit and vegetables. Another 35 schools are expected to start transforming their playgrounds soon in Aleppo and in rural areas around Damascus.

Rising prices, falling production

The price of food has risen since the start of the war — agriculture production has plummeted, and the country now relies on food imports to make up the shortfall. Transporting food around the country has also become difficult and costly.

About 13.5 million people in Syria are in need of humanitarian assistance. Of those, 7 million are unable to meet their basic food needs.

Some 5 million people receive international food aid, but not everyone in need can be reached, and the World Food Program says it has had to cut the amount of calories in its family food baskets because of funding shortages.

“The donors are generous, but we don’t know how long they can continue to be generous and rely on taxpayers’ money,” the FAO’s Yao told Reuters.

Vulnerable families are receiving help from FAO to grow food at home, so they can become less reliant on food aid.

“Food aid is very important, but … we should combine both, in a way that people grow their own food and move away from food aid gradually,” he said.

In a country where more than half the population has been forced to flee their homes, many moving several times, investing in agriculture helps people to stay put for as long as it is safe, Yao added.

“Agriculture has become a hope for [many] because they can grow their own food and survive — even in the besieged areas.”

US Attorney General: Opioid Crisis Is America’s ‘Top Lethal Issue’

U.S. Attorney General Jeff Sessions called the opioid crisis America’s “top lethal issue” Tuesday, saying that a “comprehensive antidote” was needed to address the crisis.

Speaking from the National Alliance for Drug Endangered Children national conference in Green Bay, Wisconsin, Sessions thanked the audience for their work in making the crisis’ effects on children known.

“Our country, despite the record deaths, I don’t think has fully recognized the damage this addiction nightmare is doing to us,” he said. “And as you understand this epidemic is taking a heavy toll on the most innocent and vulnerable — our children. And yet, in the national conversation about drug abuse, these children are too often forgotten.”

Sessions said that the solution has “three-pillars” — prevention, enforcement, and treatment. Sessions added that the prevention step in particular had been discussed at a meeting with top officials, including Secretary of State Rex Tillerson, and White House Chief of Staff John Kelly the day before.

Earlier this month, President Donald Trump vowed that the U.S. would “win” the battle against the heroin and opioid plague, but he stopped short of declaring a national emergency as his handpicked commission had recommended.

Wealthy Cocaine Users Funding Slavery, Says Former UK Drug Chief

Middle-class cocaine users are turning a blind eye to the link between their drug habit and sex trafficking, slavery and murder, said the former head of UK drug strategy.

“These are middle-aged, middle-class people at dinner parties,” Tony Saggers, former head of drugs threat at the National Crime Agency, told The Times on Tuesday, in his first interview since leaving the post.

“They will find sweatshops abhorrent, slave labor a brutal, terrible thing to be happening in their neighborhood, and the news that a 16-year-old has been knifed to death in London will shock them,” he added.

Britain has one of the highest rates of cocaine use in Europe, according to the European Commission and the Organization for Economic Cooperation and Development (OECD) with 4.2 percent of young adults having taken the drug in 2015.

Saggers said this was funding the exploitation of women in the sex industry, as well as slavery and gun violence.

“The consequences of buying cocaine are more abhorrent than most of what the people using it find abhorrent,” he said.

In Britain, there are an estimated 13,000 victims of forced labor, sexual exploitation and domestic servitude, most of them from Albania, Nigeria, Poland and Vietnam.

Saggers said there was a lack of action among employers in tackling the prevalence and acceptance of cocaine use in some industries, particularly among banks and other companies in the City of London, Britain’s financial center.

Companies should address the problem through schemes that educate their staff about the impact of cocaine abuse on their health and wider society, he added.

Tamara Barnett, projects leader at the Human Trafficking Foundation, said “we need to do all we can to emphasize the role the public can play in eradicating human trafficking.”

“Highlighting the exploitation and abuse behind certain drug production could make some people think twice before they purchase drugs, or at least twice before boasting about doing something that has for too long been seen just as a fashionable lifestyle choice,” she told the Thomson Reuters Foundation.

In 2015 a report by the National Police Chiefs’ Council found that commercial cultivation of cannabis was used to fund human trafficking.

Scientists Say Harvey May Be Soggy Sign of Future Storms

By the time the rain stops, Harvey will have dumped about 1 million gallons of water for every man, woman and child in southeastern Texas — a soggy, record-breaking glimpse of the wet and wild future that global warming could bring, scientists say.

While scientists are quick to say that climate change didn’t cause Harvey and that they haven’t determined yet whether the storm was made worse by global warming, they do note that warmer air and water mean wetter and possibly more intense hurricanes in the future.

“This is the kind of thing we are going to get more of,” said Princeton University climate scientist Michael Oppenheimer. “This storm should serve as warning.”

There’s a scientifically accepted method for determining if some wild weather event has the fingerprints of man-made climate change, and it involves intricate calculations. Those could take weeks or months to complete, and then even longer to pass peer review.

Storms to produce more rain

In general, though, climate scientists agree that future storms will dump much more rain than the same size storms did in the past.

That’s because warmer air holds more water. With every degree Fahrenheit, the atmosphere can hold and then dump an additional 4 percent of water (7 percent for every degree Celsius), several scientists say.

 

Global warming also means warmer seas, and warm water is what fuels hurricanes.

When Harvey moved toward Texas, water in the Gulf of Mexico was nearly 2 degrees (1 degree Celsius) warmer than normal, said Weather Underground meteorology director Jeff Masters. Hurricanes need at least 79 degrees F (26 C) as fuel, and water at least that warm ran more than 300 feet (100 meters) deep in the Gulf, according to University of Miami hurricane researcher Brian McNoldy.

Downpours on the rise

Several studies show that the top 1 percent of the strongest downpours are already happening much more frequently. Also, calculations done Monday by MIT meteorology professor Kerry Emanuel show that the drenching received by Rockport, Texas, used to be maybe a once-in-1,800-years event for that city, but with warmer air holding more water and changes in storm steering currents since 2010, it is now a once-every-300-years event.

There’s a lot of debate among climate scientists over what role, if any, global warming may have played in causing Harvey to stall over Texas, which was a huge factor in the catastrophic flooding. If the hurricane had moved on like a normal storm, it wouldn’t have dumped as much rain in any one spot.

Harvey stalled because it is sandwiched between two high-pressure fronts that push it in opposite directions, and those fronts are stuck.

Oppenheimer and some others theorize that there’s a connection between melting sea ice in the Arctic and changes in the jet stream and the weather patterns that make these “blocking fronts” more common. Others, like Masters, contend it’s too early to say.

An off-the-chart-event

University of Washington atmospheric scientist Cliff Mass said that climate change is simply not powerful enough to create off-the-chart events like Harvey’s rainfall.

“You really can’t pin global warming on something this extreme. It has to be natural variability,” Mass said. “It may juice it up slightly but not create this phenomenal anomaly.”

“We’re breaking one record after another with this thing,” Mass said.

Closing in on rainfall record

Sometime Tuesday or early Wednesday, parts of the Houston region will have broken the nearly 40-year-old U.S. record for the heaviest rainfall from a tropical system — 48 inches, set by Tropical Storm Amelia in 1978 in Texas, several meteorologists say.

Already 15 trillion gallons of rain have fallen on a large area, and an additional 5 trillion or 6 trillion gallons are forecast by the end of Wednesday, meteorologist Ryan Maue of WeatherBell Analytics calculates. That’s enough water to fill all the NFL and Division 1 college football stadiums more than 100 times over.

 

Public Health Dangers Loom in Harvey-hit Areas

The muddy floodwaters now soaking through dry wall, carpeting, mattresses and furniture in Houston will pose a massive cleanup challenge with potential public health consequences.

It’s not known yet what kinds or how much sewage, chemicals and waterborne germs are mixed in the water. For now, health officials are more concerned about drownings, carbon monoxide poisoning from generators and hygiene at shelters. In the months and years to come, their worries will turn to the effects of trauma from Hurricane Harvey on mental health.

 

At a shelter set up inside Houston’s George R. Brown Convention Center, Dr. David Persse is building a clinic of doctors and nurses and trying to prevent the spread of viruses or having to send people to hospitals already stretched thin.

“This is rapidly evolving,” said Persse, Houston Director of Emergency Medical Services. “I always worry in these large congregations of people about viral outbreaks that cause nausea, vomiting and diarrhea. And we are just getting started.”

Medics have been at the convention center since it started taking in evacuees, along with police and other first responders. Over the last 24 hours, doctors and nurses arrived at the convention center to volunteer.

 

Fewer than 20 people have been hospitalized so far from the convention center.

 

“One of our goals is to appropriately treat people here with minor things so we don’t send everybody off to the hospital,” Persse.

Many of the around 3,000 people who fled from Harvey’s flooding waited hours in water mixed with sewage, oil and gasoline. Some weren’t able to grab their medications or medical devices. Police officers on Monday afternoon were looking for more wheelchairs.

Donors have brought diapers, baby formula and other basic necessities to the convention center. Officials are working with local pharmacies to replace lost medicines and dialysis clinics to serve patients in need of treatment. The Red Cross is arranging to bring in portable showers. A shipment of 50 wheelchairs was on the way Monday afternoon.

The U.S. Environmental Protection Agency will begin sampling the floodwaters as soon as possible, said EPA spokeswoman Liz Bowman in an email Monday. EPA teams will also be visiting water and sewage plants to offer help, she said.

EPA helped secure Superfund pollution cleanup sites last week ahead of the storm and is continuing to check with site operators.

 

Floodwater can be dangerous for people with open wounds, particularly if they have other health conditions. After Hurricane Katrina, five people with infected wounds died and health officials believe that exposure to brackish floodwater contributed to the deaths.

Katrina taught other lessons.

 

“In Katrina, a lot of people were concerned about illnesses from contact with the floodwater, but more infectious disease was associated with poor hygiene in overcrowded shelter facilities,” said Karen Levy, associate professor of environmental health at Emory University in Atlanta.

At Houston-area shelters, access to clean water or hand sanitizer and proper disposal of human waste should be stressed, she said.

Any health problems will be more dangerous for the most vulnerable — people with immune disorders, the elderly and the poor, Levy said.

Katrina left a wake of post-traumatic stress disorder and depression, said Dr. Pierre Buekens, dean of Tulane University’s School of Public Health and Tropical Medicine.  He said their surveys showed roughly 10 percent had PTSD, but also that people were resilient.

“I’m sure this will happen in Houston,” he said.

The most common flood-related deaths occur when people try to drive through flooded areas, according to the Centers for Disease Control and Prevention.  Carbon monoxide may kill more as people return to homes without electricity and hook up generators, said the CDC’s Renee Funk.

“Any sort of roof over a generator is actually a problem,” Funk said. “When people go in and out to refill the generator they can be overcome. If a structure is attached to the house, the house can fill with fumes.”

Best advice: Use a battery-operated carbon monoxide detector in the house if you’re using a generator for power, she said.

Mold is also a health hazard. The CDC recommends removing and disposing of drywall and insulation that was tainted by floodwater or sewage. Mattresses, pillows, carpeting — even stuffed toys — should be tossed out. Hard surfaces can be disinfected with a solution of one cup of bleach to five gallons of water.

“That little spot of mold can grow in the home especially in the heat of the South,” said Dr. Parham Jaberi of the Louisiana Department of Health.

 

 If mold covers more than 100 square feet, a trained mold remover is recommended, he said.

 

Abuse in US Nursing Homes Unreported Despite Law

More than 1 in 4 cases of possible sexual and physical abuse against nursing home patients apparently went unreported to police, says a government audit that faults Medicare for failing to enforce a federal law requiring immediate notification.

The Health and Human Services inspector general’s office was issuing an “early alert” Monday on its findings from a large sampling of cases in 33 states. Investigators say Medicare needs to take corrective action right away.

“We hope that we can stop this from happening to anybody else,” said Curtis Roy, an audit manager with the inspector general’s office, which investigates fraud, waste and abuse in the health care system. The audit is part of a larger ongoing probe, and additional findings are expected, he said.

With some 1.4 million people living in U.S. nursing homes, quality is an ongoing concern. Despite greater awareness, egregious incidents still occur.

Using investigative data analysis techniques, auditors from the inspector general’s office identified 134 cases in which hospital emergency room records indicated possible sexual or physical abuse, or neglect, of nursing home residents. The incidents spanned a two-year period from 2015-2016.

Illinois had the largest number of incidents overall, with 17. It was followed by Michigan (13), Texas (9), and California (8).

In 38 of the total cases (28 percent), investigators could find no evidence in hospital records that the incident had been reported to local law enforcement, despite a federal law requiring prompt reporting by nursing homes, as well as similar state and local requirements.

“Based on the records we had available to us, we could not determine that they had been reported to law enforcement,” said Roy.

The federal statute has been on the books more than five years, but investigators found that Medicare has not enforced its requirement to report incidents to police and other agencies, or risk fines of up to $300,000.

Nursing home personnel must immediately report incidents that involve a suspected crime, within a two-hour window if there’s serious bodily injury. Otherwise, authorities must be notified within 24 hours.

Medicare “has inadequate procedures to ensure that incidents of potential abuse or neglect of Medicare beneficiaries residing in (nursing homes) are identified and reported,” the inspector general’s report said.

In a statement, the Centers for Medicare and Medicaid Services said “nursing home resident safety is our priority and primary focus,” and it is committed “to ensure these vulnerable people are properly cared for, and that all viable or alleged instances involving abuse or neglect are fully investigated and resolved.”

The agency said it has long required nursing homes to immediately report abuse and neglect to state officials, and it will have a formal response to the inspector general’s findings once the audit is complete.

The inspector general is urging Medicare to start systematically scouring computerized billing records for tell-tale signs of possible abuse of nursing home residents. Investigators used that approach to find the cases, matching emergency room and nursing home records.

Of the 38 unreported cases, 31 involved alleged or suspected rape or sexual abuse, about 4 out of 5.

But even among the 96 cases that were ultimately reported to police, investigators were unable to tell if the federal requirement for “immediate” notification was followed.

In one case classified as “reported to law enforcement,” an elderly woman with verbal and mobility limitations was taken to the emergency room after she was allegedly sexually assaulted by a male resident of the same nursing home. The report said two silver-dollar-sized bruises were noted on her right breast.

Nursing home staff had helped the woman bathe and change clothes after the incident. “These actions could have destroyed any evidence that may have been detected using the rape kit,” said the report.

Nursing home employees did not immediately report the incident to police, although the federal reporting requirement was in effect. The nursing home “should have reported the incident to law enforcement within two hours of witnessing the incident,” the report said.

Instead, the following day the nursing home contacted the woman’s family, who called the police, triggering an investigation.

Citing a separate probe by state officials, the inspector general’s report said the nursing home “contacted local law enforcement in an attempt to keep law enforcement from investigating the incident.”

The state’s own report found that the nursing home told police “we were required to report it but that we were doing our own internal investigation and did not need (police) to make a site visit…no one was interested in pressing charges.” The police continued their investigation.

The state later cited the nursing home for failing to immediately notify the patient’s doctor and family, as well as other violations of federal regulations. But state officials classified the incident as resulting in “minimum harm or potential for actual harm.”

No other details were provided in the federal report. The inspector general’s office reported all 134 cases to local police.

The number of nursing home residents is expected to grow in coming years as more people live into their 80s and 90s. Medicaid is the main payer for long-term care, while Medicare covers doctors’ services and hospital care for elderly people and the disabled.

Vaccine for Meningitis Shows Some Protection Against Gonorrhea

Scientists have not been able to develop a vaccine against the sexually transmitted disease gonorrhea, despite working toward one for more than 100 years.

However, they may have stumbled onto something that could provide clues to advance the development of such a vaccine.

Decades ago, in the late 1990s, a strain of meningitis B was reaching epidemic proportions in New Zealand. A vaccine, MeNZB, was developed to protect young people who were at the highest risk of getting this particular type. It did not provide protection against any other strain.

Between 2004 and 2006, MeNZB was given to anyone under the age of 20. Babies and preschoolers were routinely immunized until 2008. People with a high medical risk continued to get the vaccine until 2011. Once the epidemic was over, the vaccination program was stopped.

However, scientists noticed that the meningitis vaccine also seemed to offer some protection against gonorrhea. A study published in the Lancet last month showed that one-third of the people who had received MeNZB did not get gonorrhea, compared to a control group who was not inoculated. The lead author noted that the bacteria causing both diseases share between 80 and 90 percent of their primary genetic sequences.

Dr. Steven Black, an infectious disease expert at Cincinnati Children’s Hospital, noted, “This is the first time it’s been shown that you could have a vaccine that would protect against gonorrhea. And if these results are confirmed in another setting, that would mean that it would be very reasonable … to go forward with developing perhaps a more targeted vaccine.” Black’s comments were published in the current issue of JAMA, the Journal of the American Medical Association.

The World Health Organization reports that gonorrhea is becoming harder, and sometimes impossible, to treat, warning that it could become incurable in the not-too-distant future. At the moment, there no new antibiotics being developed to treat this disease.

“The bacteria that cause gonorrhea are particularly smart. Every time we use a new class of antibiotics to treat the infection, the bacteria evolve to resist them,” according to Dr. Teodora Wi, a medical officer involved in human reproduction at the WHO, quoted in a news release from the UN agency.

The U.S. Centers for Disease Control and Prevention (CDC) reports that gonorrhea is the second most commonly reported notifiable disease in the United States. All known cases must be reported to the CDC, but officials there estimate that they are notified of fewer than half of the 800,000 new cases each year.

Women may not have any symptoms, but untreated gonorrhea can cause serious health problems. It can lead to pelvic inflammatory disease. It can cause life-threatening ectopic pregnancies, and pregnant women can pass the disease to their babies. Gonorrhea can lead to infertility for both men and women, and can make those who have it more likely to get HIV.  

The study about the New Zealand epidemic may change the approach toward developing a vaccine against gonorrhea.

The JAMA article concludes that ultimately, a preventive vaccine could be the only sustainable solution to a fast-changing bug that has proven adept at developing resistance.
